Upon accepting the terms set out on this page, you will be redirected to the form for the action you wish to take through the Channel — Submit a report, ask a question or offer a suggestion.
Two things you need to know:- You can submit your report anonymously, without having to identify yourself;
- To track the progress of your report, write down and save the protocol number that will be generated. It will not be possible to generate a new number if it gets lost.

To complete your report successfully, you should provide as much information as possible, including any evidence you can gather. That will make it possible to validate the information in your report and take the necessary measures.
Your report may be closed without a conclusive response if you do not meet the following obligations:
- Expose the facts according to the truth;
- Act with loyalty, kindness, and good faith;
- Do not act impulsively;
- Present the information that was requested from you to clarify the facts.
